Failure to use the proper documents for the study requested.
Failure to follow the instructions that may be set out on the documents.
The documentation has not been completed properly or is illegible.
No data has been filled out in the mandatory fields marked on the documents.
Missing signature or signatures on the documents.
An inconsistency between the sample’s identification and the identification indicated on the documentation.
The samples have not been sent along with the documents.

In order to perform clinical analyses, it is essential to have the patient’s consent.
