As far as documentation is concerned, these incidents include:
- Failure to use the proper documents for the study requested.
- Failure to follow the instructions that may be set out on the documents.
- The documentation has not been completed properly or is illegible.
- No data has been filled out in the mandatory fields marked on the documents.
- Missing signature or signatures on the documents.
- An inconsistency between the sample’s identification and the identification indicated on the documentation.
- The samples have not been sent along with the documents.
In order to perform clinical analyses, it is essential to have the patient’s consent.
